Supplemental Forms and Documents


What are supplemental forms?

In addition to the standard admissions application to Dalhousie, some programs require additional information or documentation. If the program you are applying to is listed below, follow the instructions to complete the supplementary requirements.

Programs that require supplementary forms and documents:

Bachelor of Health Science

  • View supplementary requirements (Due February 15) for the following Health Sciences Disciplines: 
    • Diagnostic Medical Ultrasound
    • Nuclear Medicine Technology
    • Radiological Technology
    • Respiratory Therapy

Bachelor of Music

Bachelor of Science Nursing

  • Nursing Supplemental Form (Due February 28)
  • All applicants to Dalhousie's Bachelor of Science (Nursing) program are required to complete a 90-minute computer-based online assessment (CASPer Test) before their application will be reviewed for admission.

Bachelor of Science Pharmacy

Diploma in Health Services Administration

  • A letter outlining your life and work experiences since last attending school. You should include a description of your educational goals and your motivation to succeed in this program.
  • One letter of reference
  • Resume

Bachelor of Social Work

Diploma in Dental Hygiene

Diploma in Emergency Health Services

  • A letter outlining your life and work experiences since last attending school. You should include a description of your educational goals and your motivation to succeed in this program.

Diploma in Health Services Administration

  • A letter outlining your life and work experiences since last attending school. You should include a description of your educational goals and your motivation to succeed in this program.

Diploma in Veterinary Technology

  • Application deadline (Due February 28). If you are academically admissable, you will be invited to write an aptitude test.
