Flu vaccine safety survey

What is the purpose of this study?

The purpose of this study is to find out how often adverse events (side effects) occur after a flu shot. Information about side effects to the flu shot will be collected using a web-based survey.

What does the study involve?

You will complete two online surveys that will take about 5 minutes each.
1. Flu Vaccine Safety Survey – Complete AFTER you get the flu shot in 2017. An email will be sent eight days after your flu        shot with the link to the survey.
2. Pre-Flu Vaccine Safety Survey – Complete BEFORE you get the flu shot in 2018. An email will be sent with the link for            this survey to capture any health issues.

How do I sign up for the study?

To self-register, go to http://cirnetwork.ca/survey/halifax/ and enter the following information:

Your first name
Email address
Telephone number (daytime and evening)
Date of immunization
Which flu vaccine you recieved

If you would like to complete the survey for all family members receiving the flu shot, please complete an online registration form for each member.
