Accreditation applications are now being submitted online!

To ensure your accreditation application is processed before November 30, 2023, your application must be submitted in full by THURSDAY, NOVEMBER 2, 2023.
For pharmacy professionals
Check to see if the program you would like to accredit has already been accredited before you submit your accreditation application!

- Presented at an educational program
- Self-directed learning activities
For CE providers
- Application for accreditation for providing an educational program to pharmacy professionals
- Expedited Application for accreditation for providing an educational program to pharmacy professionals
- Renewal application (for subsequent two years after initial accreditation of program)
For Preceptors
- Preceptors for Dalhousie BSc(Pharm) and PharmD Program. CPE forms for Preceptors involved with Dalhousie Pharmacy Students would have had the CPE forms emailed to them directly after the rotation was completed If you do not have the form in your email, please contact pepadm@dal.ca
- Preceptors for Dalhousie Residency Program. CPE forms for NS Preceptors involved with the Dalhousie Residency Program should contact the Residency Coordinator directly (Nova Scotia Health Central Zone, IWK Health Centre).
- Preceptors for all non-Dalhousie Students. Please submit a self-directed learning accreditation application which can be accessed by clicking here, or using the link above for pharmacy professionals.
