This is a screening survey for those who believe they may be experiencing symptoms of a brain injury or "Havana Syndrome". There is currently an english and spanish version of this questionnaire. To complete this questionnaire, you must first email margaux.ross@nshealth.ca to obtain a confidential identification code that is required to complete the questionnaire.
