Please note: This Transcript Request Form is for students who have taken a University Preparation course through Dalhousie University College of Continuing Education. Students who have pursued a credit course of study should contact the Registrar's Office at 902-494-2450 for transcript information.

A transcript can be ordered by submitting a Transcript Request Form (available on the web, writing a letter or sending a fax to the Registration Officer, Dalhousie University College of Continuing Education. The student's signature and payment (where applicable) are required before the transcript will be released. Records will only be released with the written consent of the student.

By fax:

If you are sending a request by fax please include your signature on the request. Due to the security and integrity of the student's record, we cannot accept electronic mail requests for transcripts. The Registration Officer fax number is (902) 494-6875.

By credit card:

To pay for your request (where applicable) by credit card in addition to the above information, we require your Master Card, Visa Card or American Express Card number plus the expiry date of the card plus the cardholder's name.

A two-day rush service is offered for a fee of $15 for the first copy and $5 for each additional copy. If the request is received by 3:00pm it will be mailed or available for pickup by 3:00pm, in two working days.

If a student wishes a transcript to be sent by Fax, it is the student's responsibility to provide the correct Fax number. Please make sure ahead of time the receiver accepts faxed copies.

A transcript will NOT be processed if any University account is outstanding or if payment (where applicable) does not accompany the request.
