Supplemental Forms and Documents


What are supplemental forms?

In addition to the standard admissions application to Dalhousie, some programs require additional information or documentation. If the program you are applying to is listed below, follow the instructions to complete the supplementary requirements.

Programs that require supplementary forms and documents:

Bachelor of Arts Major in Theatre

Bachelor of Health Science

  • View supplementary requirements (Due February 15) for the following Health Sciences Disciplines: 
    • Diagnostic Medical Ultrasound
    • Nuclear Medicine Technology
    • Radiological Technology
    • Respiratory Therapy

Bachelor of Science Nursing

Bachelor of Social Work

Certificate in Magnetic Resonance Imaging

View Admission and supplementary requirements (Due March 1) for the MRI Certificate program, which include:

  • Official transcript of diploma or degree program and/or licensure.
  • Current registration number with CAMRT or applicable provincial college (or copy of license/certification).
  • Letter of Intent
  • Resume (contact references may be requested after review of application)

Diploma in Costume Studies

Applications should include a Letter of Intent containing information pertaining to any prior experience in costume related activities and reflections on “Why Costume Studies?” (1-2 pages). Also include any available photographic images of costume work. 

Diploma in Dental Hygiene

Diploma in Emergency Health Services

  • A letter outlining your life and work experiences since last attending school. You should include a description of your educational goals and your motivation to succeed in this program.

Diploma in Health Services Administration

  • A letter outlining your life and work experiences since last attending school. You should include a description of your educational goals and your motivation to succeed in this program.
